This is a modification of driverguide #119394. That download was missing some files but it was a generic version that worked with all drive manufacturers. (Most versions originally came with a new drive and will only work with drives from that manufacturer.) I copied the missing files from a manufacturer-specific version and it seems to work OK. The files I added were from version 9.56 but that doesn't seem to be a problem.
DM945 is a self extracting image file that when run will create a bootable floppy disk. The system files I used are Windows 98 first edition. When you run the file it will ask you to insert a floppy disk and then it creates the image. When you boot up using the disk it will automatically start Disk Manager.
I like it because it has both a low level format
