I found these when I went back to Win95 with HP's emergency CD, printed out the device resources, and saved the files, plus all .inf files that might be needed.
I reinstalled Win98, pointed the Modem wizard to these files, and the modem works like a charm again. This file will unzip into your My Downloads folder.
