"Windows XP home SP2 PC.

I installed a Conexant RS56 PCI modem card, and got a "PCI Simple Controller" error in the Device Manager. Tried this driver, but it didn;t fix the Controller error."

"many thanks to the poster of this driver: I was beginning to become desperate after many hours of fruitless searching!

My modem also has Ambient md5628D-L-A chipset, and letters 94V-0 stamped on the pc board.

If you get error 645 about the Microsoft dial up adapter not installed/not in use, got to: control panel, windows set-up, highlight the communications entry, cl
