I pulled these drivers off of the driver CD, I looked on the manufacturers website and you can't even get the drivers from them. So I decided to share hope this helps someone. The model number on the card is IC56A it also shows a part number of 100-08021-6 the only other number I see is on the chip it is AGERE 1648C-TV5. You can find a picture of this modem at

How to Update IC56A Device Drivers

Step 1 - Download Your Driver
